E’ uno strano momento quello della Medicina Generale in Italia! Tutti continuano a ripetere che il riordino delle cure primarie è indispensabile per la tenuta economica ed assistenziale del SSN. Noi ovviamente assentiamo, anche se resi prudenti dai risultati ottenuti negli ospedali dagli stessi assertori del cambiamento. Siccome, però, questo mitico riordino, non c’è nessuno che, rispettando i percorsi pur previsti dalla legge, si prenda la responsabilità di avviarlo, molti amministratori qua e là si sentono autorizzati al pionierismo o magari a dar sfogo a desideri frustrati, ma coltivati per anni, quando non si tratti della riproposizione sul territorio di modelli già falliti nell’universo ospedaliero. C’è comunque un’idea comune in tutto questo scoordinato fervore: quella di normalizzare la Medicina Generale, vissuta come estranea alla logica di subordinazione, di controllo anche professionale, di allocazione clientelare e come troppo libera, anzi libertaria; poco importa se tale concetto di libertà declina il principio di scelta da parte del cittadino. Si lasci pure l’etichetta a questi MMG di liberi professionisti, ma si riducano nei fatti ad una subordinazione stringente e, per il datore di lavoro pubblico, ancora più conveniente. Tuttavia un tale atteggiamento, a parte la nostra fiera avversione, qualche difficoltà la incontra nella cogenza di leggi che attualmente non sono riusciti ancora a vanificare (a parte la volontà trasversalmente diffusa, occorre tempo per farlo!) o che non si possono aggirare per via giudiziaria, anche se affidare una interpretazione a questo strumento significa, giudizio a parte, avere buone speranze di far crepare di stenti il ricorrente, cui nel frattempo si fa scontare qualsiasi arbitrio. Allora non passa giorno che qualcuno non s’inventi qualche nuova tattica, utile a raggiungere il fine della normalizzazione di cui sopra. E’ nozione comune che, anche nell’organizzazione sanitaria, conoscere i dati significa non solo legittimamente programmare, ma quando questi siano sensibili ed inerenti nominativamente a una categoria significa indirizzarla e selezionarla, non necessariamente per merito, ma magari secondo i parametri che si giudicano più opportuni. Se poi si assiste ad un conclamato matrimonio tra apicalità amministrative e scientifiche, che all’unisono, sotto l’ombrello salvifico della ricerca, si vengono ad interessare del tuo modo di essere e di comportarti e neppure soltanto professionale e ti invitano a segnalarlo e ad essere segnalato con tanto di nomi e cognomi in una ristretta comunità professionale, ovviamente, non ti viene neanche in mente che il datore di lavoro predisponga meccanismi di selezione del personale come ti verrebbe in mente se le stesse domande te le facesse, ad esempio, il responsabile del tuo DCP.
Eppure il Legislatore, il Garante della privacy hanno attentamente ristretto e tutelato l’acquisizione di dati sensibili da parte di chicchessia: pubblica amministrazione o istituzione scientifica.
Ma la scienza è la scienza! E chi obbietta, come minimo, si aspetti con tanto di inarcamento sopraccigliare l’etichetta del rozzo ignorante, naturalmente attraverso perifrasi politicamente corrette. Pazienza se manca a garanzia dell’indagine un comitato etico noto, se non è richiesto il consenso ai partecipanti, se l’informativa agli stessi è più un ordine di servizio che altro, se per quel poco che si sa il rilievo scientifico dei dati richiesti non muterebbe se acquisiti in forma anonima anziché attraverso giudizi codificati, personalizzati con tanto di nomi e cognomi del giudicante e dei giudicati.
Che importa è ricerca! Intanto poi, ripeto poi, i dati saranno aggregati, il prima che conta? Siamo scienziati, autoreferenzialmente edotti, di tutto ovviamente.
Noi che siamo MMG, invece, “sappiamo di non sapere”, ma aspiriamo sempre più a sapere; sarà per questo che privilegiamo continuamente il confronto, la contaminazione delle idee, le condivisioni possibili, i comportamenti consapevoli? E’ proprio vero, in questo SSN ci meritiamo di essere normalizzati.
