La malattia diverticolare (MD) ha una prevalenza superiore al 40% nel mondo occidentale soprattutto nella fascia dei 60 anni; circa il 25% dei casi non è diagnosticato, inoltre tra il 10% e il 25% dei casi sviluppa complicanze .Si possono individuare tre categorie cliniche per la MD:
- 1. Asintomatica, diagnosticata attraverso clisma doppio contrasto del colon o colonscopia;
- 2. Sintomatica non complicata con o senza irregolarità dell’alvo;
- 3. Sintomatica complicata (rettorragia, peridiverticolite, ascessi, ostruzioni, ecc.).
Spesso i sintomi della MD tipo 2 sono indistinguibili da quelli della sindrome dell’intestino irritabile (SII), sebbene non ci siano dati a sostegno che la SII possa essere precursore della MD.
Gli obiettivi principali della gestione del paziente con MD sono il buon controllo della sintomatologia addominale e la prevenzione della diverticolite acuta e delle complicanze. Il Medico di Medicina generale è quasi sempre il professionista che per primo riscontra la MD nella propria pratica clinica quotidiana, soprattutto nei pazienti anziani dove i sintomi sono più subdoli.
