Il 5 e 6 maggio scorsi a Roma, presso il Centro Congressi Cavour, si è tenuto il Congresso "La gestione moderna dello Scompenso Cardiaco", a cui hanno partecipato numerosissimi Medici di Medicina Generale. Il dibattito che si è realizzato in aula ha messo in evidenza quanto appaia sempre più importante il coinvolgimento della Medicina Generale nell'approccio a questa patologia sia da un punto di vista professionale-scientifico che organizzativo.
Lo scompenso cardiaco è una patologia complessa ad elevata prevalenza, stimata tra l’1-10% nella popolazione generale, e oltre il 10% nella popolazione sopra i 70 anni di età, ad alta morbilità e mortalità, tanto da costituire un’importante problematica in termini di ricerca di nuovi modelli di management nell’ambito del servizio sanitario nazionale.
In particolar modo negli ultimi decenni si è assistito ad un notevole aumento dell’incidenza dello scompenso cardiaco e del numero di ospedalizzazioni ad esso correlate, aumento dovuto in parte al costante allungamento della vita media ed alla crescente efficacia dei trattamenti farmacologi e non.
Al momento attuale si calcola che la spesa sanitaria per la cura dello scompenso cardiaco si aggiri intorno al 2% della spesa sanitaria complessiva, spesa aggravata soprattutto dalle ripetute riospedalizzazioni dei pazienti con scompenso cardiaco. E’ evidente che tale situazione sia causata dall’assenza di un valido ed uniforme piano di gestione del paziente con scompenso. Alla luce di questi dati si evince la necessità di un modello di gestione di questa patologia cronica volto alla riduzione della mortalità, al miglioramento della qualità di vita del paziente, concernente soprattutto la riduzione del numero di ricoveri, e la conseguente riduzione della spesa sanitaria.
L’importanza in ambito sanitario dello scompenso è destinata a crescere progressivamente e necessita di nuove strategie assistenziali volte a :
- Ridurre la mortalità
- Migliorare la qualità di vita
- Contenere la spesa sanitaria
I punti cardine nella formulazione di un modello gestionale “vincente” sono:
- Ottimizzazione delle strategie di intervento (farmacologico e non) volte ad eliminare ridondanze terapeutiche e diagnostiche con spreco di risorse
- Trattamento multidisciplinare, in cui la comunicazione e l’interazione tra specialista MMG e il personale infermieristico sia l’aspetto centrale
- Trattamento personalizzato per un follow up specifico e ad hoc per il singolo paziente
- Organizzazione di servizi sul territorio integrati e coordinati con le strutture ospedaliere ed extra ospedaliere.
