- 1) Rivalutare criticamente tutti i farmaci assunti dal paziente e le motivazioni della assunzione;
- 2) Considerare il rischio complessivo di danno indotto da farmaci nei singoli pazienti per modulare la deprescrizione;
- 3) Valutare per ciascun farmaco il rapporto rischio/beneficio;
- 4) Dare priorità alla sospensione di farmaci che presentano il rapporto beneficio/rischio più basso e la più bassa probabilità di reazioni avverse da sospensione o di sindromi da rebound della malattia;
- 5) Implementare un regime di interruzione e monitorare attentamente i pazienti per migliorare i risultati o prevenire l'insorgenza di reazioni avversi.
Con l’aumento dell’aspettativa di vita è aumentato nella popolazione il numero dei soggetti ultrasettantacinquenni. L’aumento dell’età si correla spesso ad un aumento di pazienti affetti da più patologie a cui corrisponde un aumento del numero di farmaci assunti: secondo il rapporto OSMED 2017 1 nella fascia di età 65-69 anni il numero di principi attivi utilizzati è pari a 7.7, che passa a 11.8 nei pazienti con età ≥85 anni. L’analisi dei dati ha evidenziato inoltre che il 21.6% dei soggetti di età ≥65 anni assume almeno 10 principi attivi diversi dimostrando in questa popolazione un frequente ricorso alla politerapia. Le classi terapeutiche maggiormente prescritte in questa popolazione sono quelle dei farmaci per l’apparato cardiovascolare, antimicrobici per uso sistemico, farmaci per l’apparato gastrointestinale e per il metabolismo. Se analizziamo la prevalenza d’uso, i più prescritti sono i farmaci usati nell’ulcera peptica e nel reflusso gastroesofageo (48.3%, 47% uomini e 49.2% donne), seguiti dagli antitrombotici (41.5%, 45.2% uomini e 38.6% donne), dalle sostanze modificatrici dei lipidi (34.4%, 36.9 uomini e 32.6 donna), e dai FANS (32.8%, 28.6% uomini e 32.6% donne). Con l’aumentare del numero dei farmaci assunti vi è un corrispettivo aumento del rischio di interazione fra farmaci, cioè la modificazione degli effetti di un farmaco indotta dalla presenza di un altro farmaco e soprattutto del rischio di reazioni avverse (ADRs). L’aumento del rischio di ADRs è correlato al numero dei farmaci assunti, se sono ≥7 è pari all’82% e per ≥10 è praticamente del 100% 2. Ciò di fatto porta con sé un peggioramento della qualità di vita ed un aumento della morbilità e della mortalità; nei paesi industrializzati le ADRs rappresentano la 4° - 6° causa di morte 3 e da uno studio prospettico condotto nel 2005 presso 22 PS nel nostro paese è risultato che, su 18.854 accessi, 629 (il 3,3%) erano dovuti a queste 4. Oltre a ciò, nei pazienti anziani si verificano modificazioni età-correlate nei parametri farmacocinetici e farmacodinamici rispetto alla popolazione più giovane 5. Un’altra criticità legata alle polipatologie è che i pz vengono assistiti in maniera spesso discontinua e frammentaria da più figure professionali, con conseguenti problemi di comunicazione e coordinamento riguardo le terapie. Bisogna inoltre considerare che le prescrizioni terapeutiche, anche se fatte nel rispetto delle più accreditate linee guida, non sono sempre adatte al paziente anziano, perché generalmente gli studi che le supportano quasi mai includono pazienti anziani con pluripatologie e politerapie ed in genere gli studi riguardano un solo farmaco 6. Sulla base di quanto detto appare chiaro che la prescrizione di farmaci nel paziente anziano è un fenomeno molto complesso nel quale bisogna agire con grande attenzione e preparazione per ridurre al minimo i rischi legati alle inappropriatezze, prescrizione di farmaci non indicati, ingiustificati, prescrizioni duplicate, cascata prescrittiva, come pure mancata prescrizione di farmaci indicati. Per agevolare questo lavoro è possibile avvalersi di una serie di indicatori presenti in letteratura. Tra i più utilizzati a livello internazionale sono i criteri di Beers e i criteri STOPP & START. I criteri di Beers 7, che devono il nome al geriatra americano che per primo li pubblicò nel ’91, sono stati il primo set di indicatori di inappropriatezza, rivisti e rielaborati più volte negli anni; identificano farmaci o classi di farmaci che dovrebbero essere evitati negli ultrasessantacinquenni o più, perché inefficaci o comportanti un rischio inutilmente elevato, o da non utilizzare in particolari condizioni cliniche. Nell’ultima revisione (2015) sono stati aggiunti farmaci che possono dare interazioni pericolose, farmaci controindicati nei soggetti con funzionalità renale ridotta ma soprattutto sono state fornite liste di farmaci alternativi ed opzioni di trattamento non di tipo farmacologico. I criteri STOPP & START (ultima edizione nel 2015) 8, sono degli indicatori che
permettono di identificare prescrizioni e farmaci potenzialmente inappropriati nel paziente anziano (per Screening Tool of Older Person’s Prescriptions), comprendenti interazioni farmaco-farmaco, farmaco-malattia, durata e dosi di trattamento e, per Screening Tool to Alert doctors to Right Treatment, potenziali errori di omissione di farmaci che sarebbero invece da prescrivere. Da tutto ciò deriva la necessità per i MMG di un diverso approccio al pz anziano che sia orientato specificatamente alle problematiche legate alla multimorbidità. Innanzi tutto i farmaci utilizzati devono essere appropriati per l’età dei pazienti, quindi bisogna ridurre o eliminare minimizzare l’uso di farmaci potenzialmente inappropriati, infine deprescrivere quando possibile, farmaci dall’efficacia dubbia o nulla e dal profilo rischio beneficio sfavorevole per il paziente.
La deprescrizione è tra le diverse misure adottabili, ed è importante avere una maggior consapevolezza su quello che è stato definito come "un processo sistematico di identificazione e sospensione di farmaci o regimi farmacologici in circostanze in cui evidenti o potenziali effetti negativi superino i benefici presenti e/o potenziali, tenendo conto degli obiettivi di cura, del livello di funzionamento, dell’aspettativa di vita, dei valori e preferenze del singolo paziente".
Purtroppo non ci sono delle linee guida che forniscano raccomandazioni per decisioni così complesse e difficili, ma Scott I.A. e coll. 9 ed altri autori (Cartabellotta A) 10 hanno formulato una guida intesa come un approccio proattivo alla sospensione dei farmaci nell’anziano. Per procedere alla deprescrizione bisogna prima di tutto effettuare una ricognizione completa di tutti i farmaci assunti dal paziente, compresi fitofarmaci, farmaci omeopatici e farmaci da banco. Successivamente bisogna considerare il rischio complessivo, valutando il numero dei farmaci (³ 8 farmaci), l’età (≥75 anni) ed i farmaci a rischio elevato di ADRs. In questi pazienti l’obiettivo è di ridurre i farmaci a 5 o meno di cinque, interrompendo quelli per i quali vi è ragionevole certezza di tossicità 10. Si deve valutare quindi per ciascun farmaco il rapporto rischio/beneficio e dare priorità all’interruzione di quelli il cui rischio assoluto di eventi avversi è maggiore dei benefici e di quelli con la più bassa probabilità di reazioni avverse da sospensioni o da effetti rebound della malattia. Di fondamentale importanza in questa operazione è avere il consenso del paziente e del suo care-giver e riconciliare i farmaci da sospendere con le preferenze del paziente.
Per facilitare il processo di deprescrizione I.A. Scott 11 propone un algoritmo in 5 punti, di seguito elencati:
- 1) Rivalutare criticamente tutti i farmaci assunti dal paziente e le motivazioni della assunzione;
- 2) Considerare il rischio complessivo di danno indotto da farmaci nei singoli pazienti per modulare la deprescrizione;
- 3) Valutare per ciascun farmaco il rapporto rischio/beneficio;
- 4) Dare priorità alla sospensione di farmaci che presentano il rapporto beneficio/rischio più basso e la più bassa probabilità di reazioni avverse da sospensione o di sindromi da rebound della malattia;
- 5) Implementare un regime di interruzione e monitorare attentamente i pazienti per migliorare i risultati o prevenire l'insorgenza di reazioni avversi.
Il MMG non ha materialmente il tempo da dedicare alla valutazione del paziente anziano con pluripatologia e politrattato e spesso non ha neanche il bagaglio di conoscenze necessarie per un tale approccio. Da ciò deriva la necessità di fare self audit (ovviamente non durante il normale orario di lavoro) e di una formazione specifica, anche universitaria, con corsi dedicati.
E' fondamentale condividere il processo valutativo e le decisioni terapeutiche con il pz, i familiari e il care-giver e possibilmente i colleghi che a vario titolo intervengono nella cura del paziente.
E' indispensabile uno stretto monitoraggio dell’aderenza terapeutica e dell'insorgenza dii eventuali effetti collaterali e ADRs.
Esistono già in letteratura linee guida di pratica clinica per la deprescrizione di alcune classi di farmaci, con relativi algoritmi. Di seguito vengono descritte alcune di queste.
